Overview: State Care Coordination is an activity designed to improve recovery outcomes for individuals identified as high risk for relapse. This individual is responsible for providing case management services, discharge planning, and care coordination for individuals transitioning from an American Society of Addiction Medicine (ASAM) Level Residential Substance Use Disorder Treatment (SUD) Program (3.7, 3.5, 3.3, 3.1). This person will have a caseload of clients to aid them in achieving adjustments to specific barriers and situations of a difficult or complex nature by connecting them with the appropriate community resources.  Duties may include, but not be limited to providing intake services to clients; preparing reports, treatment plans, discharge/continuing care plans, and keeping necessary records within client database and electronic health record; coordinating the activities of one or more groups of clients; and generating referrals and aftercare plans. Work is performed in accordance with established regulations, and policies and procedures; however, this individual is expected to exercise considerable initiative and independent judgment in achieving objectives. Work will be reviewed by the Program Director, Program Supervisor, or Clinical Supervisor through the completion of assignments.

Responsibilities

What you'll do at Gaudenzia:

 

  1. Responsible for conduction intake interview and orientation to Care Coordination with clients into Care Coordination 1-3 days after referral is received.
  2. Responsible for liaison with clinical staff for review and verification of recommended services, referrals to clinically recommended level of care, and collateral contact information.
  3. Completion of Recovery Plan in coordination with client and primary counselor.
  4. Responsible for identifying healthcare and recovery support needs with client and initiating referrals
  5. Responsible for assisting clients in getting to the next level of care by removing barriers, providing reminders, and conducting outreach.
  6. Responsible for developing and maintaining relationships with services and programs. These linkages will include but are not limited to treatment, healthcare, employment, legal housing, child welfare, social services, peer support providers, etc.
  7. Establish and maintain Care Coordination services and contact schedules
  8. Maintain client records
  9. Provide follow up contact to client at least twice a month for client progress and/or obstacles in their recovery
  10. Responsible for reporting monthly activities to the Program Director, Program Supervisor, or Clinical Supervisor
  11. Responsible for maintaining community agreements, may also include but not limited to completing onsite check list, inspections and walk through for new agreements.
  12. Complete progress notes for all client-related activities within 48 hours of service delivery
  13. Other duties as assigned by Supervisor

Qualifications

Qualifications for this Role:

 

Education and Training:  A minimum of Associate Degree (Bachelors preferred) in a Human Service field or full certification as an Addictions Counselor.

 

 

Experience:                           Associates Degree:                                            Five (5) years

                                                Bachelors Degree:                                             Three (3) years

                                                Masters Degree                                                 Two (2) years

                                                Certified Addictions Counselor (CAC)            Five (5) years
