Western Pennsylvania.  Programs include Withdrawal Management, MAT, Residential and Outpatient.  FNP/PMHNP preferred

 

 

Job Overview

 

Responsible for providing health services through advanced nursing practice in a specialty appropriate to the population served by Gaudenzia, and has the authority for the prescribing, dispensing and administration of medical and addiction services including all levels of ASAM withdrawal management and medical monitoring in accordance with all applicable federal and state laws and regulations. Specialty areas may include women’s health, behavioral health, maternal/child services, HIV/AIDS, or a related area. Services may include: complete physical or mental health examinations, medical/health treatment planning, including comprehensive case management of medical/health services, diagnosis and treatment in areas of clinical specialty, such as HIV/AIDS, sexual health and reproductive issues, infection control, health education and advocacy.

 

Work is performed with considerable independence and in accordance with established regulations, policies, procedures and standards of advanced nursing practice. Efforts are coordinated with and under the direction of the Program Director(s) in program(s) where clients are being served, with identifiable goals and program planning. Professional activities are conducted under the supervision of and in coordination with the Program Director(s), Consultant Physician and/or Director of Health Services. This individual is responsible to the Program Director(s) and/or Chief Medical Officer

Responsibilities

Duties and Responsibilities

  1. Adheres to the vision and mission of the company.
  2. Promotes the utilization of all types of medication assisted treatment (MAT)
  3. Provide detox/MAT support and education as needed
  4. Utilizes Gaudenzia’s approved pharmacy
  5. In conjunction with the Clinical Supervisor, Nurse Manager, Program Director and Medical Director monitors all types of facility discharges to ensure appropriate care coordination is completed.
  6. In conjunction with the Program Director and Medical Director, ensures that a 24-hour emergency contact is available. 
  7. Attends treatment team meetings on a monthly
  8. Attends quarterlymedical staff meeting.
  9. Assists with monitoring facility incident reports and staff training as requested. 
  10. Maintains safe, secure and healthy work environment by following and enforcing standards and procedures as per company policy and federal regulations.
  11. Ensures that significant adverse drug reactions are reported to the Federal Food and Drug Administration and to the manufacturer in a manner that does not violate the client’s confidentiality
  12. Review and certify client admissions.
  13. Conduct physical examinations within timeframe specified by Company policies and procedures, and licensing and regulatory entities.
  14. Ensures all information obtained during the course of physical examinations, laboratory results and other diagnostic evaluations are duly recorded in all patient records
  15. Manage client medical care.
  16. Maintains confidentiality of all clients
  17. and interpret pre-existing medical examination reports to include but not limited to diagnosis and treatment recommendations.
  18. Order and monitor medication and medical care for clients as needed and provide written authorization.
  19. Complete and review client documentation contemporaneously in the Company’s electronic health record (EHR) as required, including but not limited to physical examinations, medication orders, physicians’ notes, nursing and biopsychosocial assessments, treatment plans and discharge summaries within twenty-four (24) hours and no later than seventy-two (72) hours of service provision.
  20. Conduct Peer-to-Peer reviews as required with funding/referral sources.
  21. Meet with the Program Director and Quality Assurance Team and participate in quality and performance improvement initiatives and peer review activities, as needed.
  22. Participate in multidisciplinary team meetings, facility meetings, and conduct education for staff and clients on substance use and mental health symptoms, treatment, and medication.
  23. Provide crisis intervention in situations requiring immediate response.
  24. Adhere to all applicable Company policies and procedures as well as licensing/ accreditation standards.
  25. Maintain valid licenses and current insurance for the length of time this contract remains in place or longer if required by the malpractice insurance carrier to cover losses or potential losses that occur(ed) during the time of this contract.
  26. Deliver telemedicine services in compliance with all regulatory entities, as requested.
  27. Create a plan for medical/health services throughout the duration of client’s treatment.
  28. Provide comprehensive case management services to ensure continuity of care.
  29. Foster efficient and effective utilization of existing medical resources to prevent fragmentation and duplication of services.
  30. Provide extensive and focused health/mental health education for clients, tailoring the information and approach to the needs of the population served.
  31. Provide individual/group counseling and education in areas of medical/clinical/health needs.
  32. Provide professional in-service training and education to staff.
  33. Create and maintain thorough and comprehensive medical/health related files on clients served.
  34. Maintain working area within appropriate and relevant regulatory guidelines, including medication handling and storage.
  35. Assist in facilitation of special events as required.
  36. Provide on-call support for medical staff
  37. Other duties as required/assigned by Chief Medical Officer

 

 

Required Knowledge, Skills and Abilities:

  • Knowledge of state regulations for drug and alcohol programs as well as opioid treatment programs. Prior experience and familiarity treating opiate and alcohol addiction with MAT is required.  Must have ability to perform physical examinations in person and using telemedicine. Knowledge of specific health care issues pertaining to drug and alcohol abusers.
  • Knowledge of current relevant advanced nursing practice issues and standards.
  • Knowledge of record keeping requirements, as well as other regulations of entities having responsibility for licensing, approving and credentialing the program served.
  • Knowledge of physical and mental health issues of the subculture served.
  • Knowledge and skills relevant to the area of advanced nursing practice.
  • Knowledge of the specialized population/area of service.
  • Knowledge of the external network of resources needed.
  • Ability to perform highly specialized and advanced nursing assessment and practice.
  • Ability to establish and maintain effective professional working relationships within and outside of the agency.
  • Ability to exercise professional judgment at the level of clinical excellence and advanced practice.
  • Ability to assess and develop educational programs for the needs of clients and staff.
  • Knowledge of specific health care issues pertaining to drug and alcohol abusers.
  • Knowledge of current relevant advanced nursing practice issues and standards.
  • Knowledge of record keeping requirements, as well as other regulations of entities having responsibility for licensing, approving and credentialing the program served.

Qualifications

Minimum Requirements

Education/Training: 

  • Qualified provider must have a Drug Addiction Treatment Act 2000 (Data2000) waiver for office-based opioid treatment (OBOT). Must have completed an accredited training program and have a state license in good standing.  Must be a health care provider in good standing with Medicare, Medicaid, and other federal and state health insurance programs, i.e. not excluded from participation in Medicare, Medicaid or any other federal or state health insurance program.
  • Nurse Practitioner program, advanced Clinical Specialist standing or other advanced practice certification.
  • FNP/PMHNP preferred

 

Experience: 

  • Qualified provider will have experience providing multiple types of withdrawal management, psychiatric medication management and medication assisted treatment (MAT) in multiple settings
