Who We’re Looking For

 

A Program Coordinator who plans and coordinates multiple programs within a facility. Their responsibility is to make sure the program is functioning efficiently and effectively. A Program Coordinator works closely with the Program's Supervisors and Directors to make sure the program is following policies/procedures. This position creates an environment of open communication within the program by collaborating with other departments on decision-making responsibilities and initiating new goal-setting initiatives that are in the best interest of the clients served.

Responsibilities

Your responsibilities in this role may include:

 

  • Ensure the clinical/therapeutic approved group schedule is adhered to daily
  • Ensure that all new clients are acclimated and orientated to the program satisfactorily
  • Responsible for the coordination of the aftercare component, organizing and confirming next level follow-up
  • Attend all applicable meetings as directed
  • Proficient in Gaudenzia's policies/ procedures inclusive but not limited to WARMS/Detox/lCF
  • Ensuring and utilizing appropriate interventions and conflict resolution of client behaviors
  • Ability to establish and maintain effective working relationships with clients, other staff members, outside agencies/ institutions, stakeholders, and the general public
  • Communicate daily with outside agencies accordingly when applicable
  • Complete and communicate all SOR documentation timely, including SOR Referrals and SOR Discharge forms to the billing team
  • Oversee the County Crisis Walk-in Data Spreadsheet
  • Complete required (2) County reports due on the 8th of each month which also includes submitting a success story to the Health Department
  • Assist the Director or designee in any preparation needed for all audits for assigned programs
  • Assist the Director or designee in ensuring that all programs are in compliance
  • Assist the Director or designee in all quantitative/qualitative non-clinical chart audits
  • Educate clients on Gaudenzia's programs available
  • Assist and oversee the transition process and collaborate with the treatment team daily
  • Monitor the process of admissions, transitions, and discharges through the continuum of care
  • Devise evaluation strategies to monitor performance and determine the need for improvements
  • Assist with support staff trainings as deemed applicable for new and existing staff
  • Knowledge of basic principles, practices and procedures involved in Gaudenzia, Inc.
  • Knowledge of the physical and emotional characteristics and manifestation of drug dependency.
  • Considerable knowledge of individual and group behavior and the effective ways of working with specialized populations.
  • Ability to exercise judgment and discretion in implementing, interpreting and evaluating program policies, procedures and regulations

Qualifications

Qualifications for this role:

 

Education/Experience: Must hold a Master’s degree plus 2 years’ experience or BA/BS or CAC plus 3 years’ experience or AA/AS plus 4 years’ experience from an accredited college or university in relevant health or human services.
