With your help, we can turn the tide on substance use disorder.       Who You Are You are a Child Care Work who is passionate about helping individuals suffering from substance use disorders and/or mental health illnesses.  Your responsibility will be to see that all the needs of the children in the facility are being met while the mother is involved in the various stages of treatment. These needs will include emotional and physical, and the approach is to be geared specifically to the individual child. The Child Care Worker assists the mother in gaining a greater understanding of the child, of childcare and development, and the relationship of mother and child. Duties may include, but are not limited to: being responsible for developing and implementing activities for those children who are too young to attend public school. You  will be required to teach the child basic skills, how to care for him/herself, keep necessary records, participate in group counseling activities, work with community agencies and provide necessary support for mother and child in reference to parenting and education about child development. Work is performed in accordance with established regulations, policies and procedures, but the person is expected to exercise considerable initiative and independent judgment in achieving objectives. You will be required to pass all clearance checks as required by Act 33 of the Child Protection Services Law.

Pay Grades for this job varies between grades 9-10 (depending on years of experience)   We are offering a for this position!   This individual will be responsible for providing the initial nursing assessment on all new admissions, utilizing the required assessment forms within the electronic health record system, and following up on all clients during treatment.  This individual will also be responsible for arranging necessary visits with Health Care Providers to complete the initial physical exam and other health screenings as required by regulations for each level of care. Other duties will include, but not be limited to, administering medications and recording this activity in the appropriate medical chart; arranging and coordinating all medical, dental, and vision exam appointments; ensuring there are adequate medical supplies; and securing necessary medical records on all clients.   Work is performed with considerable independence in accordance with established regulations, policies and procedures of the agency.  Work is also performed in accordance with the Nurse Practice Act and under the direct supervision of a Registered Nurse (where applicable), in a coordinated effort with the Program Director, Nurse Manager and/or Director of Health Services

Agency sanitation and Health Department standards must constantly be observed. This individual is responsible for providing case management services, discharge planning, and care coordination for individuals transitioning from an American Society of Addiction Medicine (ASAM) Level Residential Substance Use Disorder Treatment (SUD) Program (3.7, 3.5, 3.3, 3.1). This person will have a caseload of clients to aid them in achieving adjustments to specific barriers and situations of a difficult or complex nature by connecting them with the appropriate community resources.  Duties may include, but not be limited to providing intake services to clients; preparing reports, treatment plans, discharge/continuing care plans, and keeping necessary records within client database and electronic health record; coordinating the activities of one or more groups of clients; and generating referrals and aftercare plans. Work is performed in accordance with established regulations, and policies and procedures; however, this individual is expected to exercise considerable initiative and independent judgment in achieving objectives.
