This person may handle an independent caseload and assist other personnel by ensuring proper referral of clients to all appropriate services and resources available and necessary to them. These services and resources may include medical care, benefits, education and employment, housing, recovery and prevention services, and aftercare supports.   Duties may include, but not be limited to providing intake services to clients; preparing reports, treatment plans, discharge/ continuing care plans, and keeping necessary records within client database and electronic health record; coordinating the activities of one or more groups of clients; and generating referrals and aftercare plans. Work is performed in accordance with established regulations, and policies and procedures; however, this individual is expected to exercise considerable initiative and independent judgment in achieving objectives.
