With your help, we can turn the tide on substance use disorder.       Overview: State Care Coordination is an activity designed to improve recovery outcomes for individuals identified as high risk for relapse. This individual is responsible for providing case management services, discharge planning, and care coordination for individuals transitioning from an American Society of Addiction Medicine (ASAM) Level Residential Substance Use Disorder Treatment (SUD) Program (3.7, 3.5, 3.3, 3.1). This person will have a caseload of clients to aid them in achieving adjustments to specific barriers and situations of a difficult or complex nature by connecting them with the appropriate community resources.  Duties may include, but not be limited to providing intake services to clients; preparing reports, treatment plans, discharge/continuing care plans, and keeping necessary records within client database and electronic health record; coordinating the activities of one or more groups of clients; and generating referrals and aftercare plans. Work is performed in accordance with established regulations, and policies and procedures; however, this individual is expected to exercise considerable initiative and independent judgment in achieving objectives. Work will be reviewed by the Program Director, Program Supervisor, or Clinical Supervisor through the completion of assignments.

The program provides services for elementrary, middle and high school age teens, economically disadvantage youth, children of Substance Abusers, parents, family members, teachers, civic groups, coalitions, community and the general population.  Monthly parent and family education groups are offered and participation is encouraged.  The program offers 8 and 12 week cycles for after school and in-school programing Peer leaders and Camera/Video interviewers participate throughout the school year and or summer programing utilizing the ARISE/Too good for Drugs curriculums.  Hours would be Monday - Friday. You have the ability to help those in treatment with respect and dignity, and by providing quality services. You are trust worthy, empathetic, adaptable, compassionate and a great listener. You are a problem solver and is not afraid to have difficult conversations. You have the ability to positively influence the vulnerable. You have good interpersonal and collaboration skills, with the ability to work successfully with people at all levels.  You are capable of performing and supervising work in the therapeutic care, treatment, and rehabilitation of persons within the program.   You want to join an experienced, supportive team who is also passionate about helping individuals suffering from addiction and mental illness, achieve a better quality of life, sobriety, and abstinence from substance use/abuse.
